Forms & Fees

Hello Swimmers and Parents,

All grades, with the exception of the Seniors, need to complete the waiver form located on CHS's website. "It is required that the orientation be reviewed by a parent/guardian and student and the waiver  is signed by a parent/guardian upon completion of that review. The signed waiver  should be given to the Activities Department. No incoming 9th grader or new student may participate in the Athletics program without performing these two actions."

4. Watch Power Point
5. Click wavier link and print form.  *must be signed by parent or guardian
6. Turn in form to Activities Department.   


All Forms & Fees need to be turned in by Aug. 20th in order to swim at the Jamboree @ Superior on Thursday, Aug 21st @ 5PM.
